Mindfulness

Mindfulness has its roots in Buddhist contemplative traditions, where it is practised as a fundamental component of the path to liberation from suffering. Jon Kabat-Zinn adapted these practices to the Western clinical context in 1979, creating the Mindfulness-Based Stress Reduction (MBSR) programme. His secular approach allowed meditation to be integrated into hospitals and clinics, defining mindfulness as paying attention in a particular way: on purpose, in the present moment, and non-judgementally.

Mindfulness has been integrated into several evidence-based psychological therapies. Mindfulness-Based Cognitive Therapy (MBCT) combines cognitive elements with meditation to prevent depressive relapse. Acceptance and Commitment Therapy (ACT) uses mindfulness to foster psychological flexibility, while Dialectical Behaviour Therapy (DBT) includes mindfulness skills as a core module. These integrations have significantly broadened the clinical reach of contemplative practices.

Neuroscientific research has identified brain changes associated with regular mindfulness practice. Neuroimaging studies show alterations in the prefrontal cortex, insula, and amygdala — regions linked to emotional regulation and interoceptive awareness. Improvements in sustained attention, emotional regulation, and reduced stress reactivity have been documented. However, many of these studies have methodological limitations, including small sample sizes and the absence of active control groups.

Criticism of mindfulness includes the phenomenon known as 'McMindfulness', which denounces its commercialisation and decontextualisation from its original ethical roots. Some researchers warn that the practice can be counterproductive in certain cases, triggering anxiety or derealisation in vulnerable individuals. Furthermore, the meta-analysis by Goldberg and colleagues concluded that while mindfulness is effective, it is not clearly superior to other active treatments. This suggests that extraordinary claims about its benefits need to be tempered.
